New
Health Requirement for Kindergarteners: Eye Exam
An eye examination by an ophthalmologist or an optometrist is required
for students entering Kindergarten (within 1 year prior to entry). Proof
of the eye examination on the approved State
of Illinois Eye Examination Report form with the signature of the eye doctor is required before the
first day of school. These are included in the registration packets.
The McLean County Health Dept has an eye clinic available to children
in McLean County who meet income criteria. Call 309/888-5479 for more
information. Students who qualify for eye care through the Illinois Dept
of Public Aid are not eligible for the MCHD eye clinic. Students that
possess a Public Aid medical card may call Scott Health Resources at
309/823-4224 for a referral if needed.
